Who am I?

FAI Bone Shapes Don’t Consistently Predict Restriction, Arthritis, or Hip Pain

After imaging, you may be told you have cam, pincer, or mixed impingement morphology. And you may have heard this is definitively linked to labral tears, arthritis, movement restrictions, and hip pain.

But research findings paint a more complex picture.

Multiple studies have found limited or inconsistent correlations between these structural findings and hip pain, movement limitations, or the later development of arthritis.

Research has also shown these bone shapes to be highly prevalent in asymptomatic populations.

And many people have hip impingement bone shapes without ever experiencing hip mobility or pain problems -- even into old age!

Labral Tears Don't Consistently Predict or Explain Pain Either 

Labral tears are frequently found on imaging, even in people without hip symptoms. In some studies, the majority of people with no hip problems have 'abnormalities,' which suggests labral tears in an MRI do not always explain hip pain or movement restriction.  

Why Hip Impingement Tests Can Be Misleading

Clinical “special tests” are commonly used to evaluate hip impingement and labral pathology, but research findings regarding their diagnostic accuracy are mixed.

Studies have found that these tests can have low specificity and relatively high false-positive rates. This means positive findings don’t always correlate with symptoms or structural pathology.

A large 2020 systematic review concluded: “Due to the low specificity of clinical tests, we would not recommend their use to confirm the diagnosis of FAI syndrome … there is too high uncertainty due to low-quality evidence and high risk of bias.”

Hip Injections Don't Seem to Predict Surgical Outcomes

Surgeons sometimes use anesthetic hip injections to determine whether symptoms are coming from inside the joint. The beIief is that if an injection reduces pain, surgery targeting that area may be beneficial.

However, research on this predictive relationship doesn't seem supportive. Multiple studies have found that pain relief from diagnostic injections did not consistently predict surgical outcomes.

What the Research Actually Says About FAI Surgery Results  

This is the most important thing to know. Success rates are often presented as high, but research shows more mixed results when you look at patient satisfaction.

One early study reported disappointment rates as high as 56%.

A 2021 study examining patient satisfaction found fewer than half of patients were satisfied with their surgical outcomes.

A randomized controlled trial found surgical results were not significantly different from those achieved through conservative exercise-based care.

And surgery does not even guarantee you'll preserve hip cartilage. A randomized trial that measured MRI cartilage changes found more cartilage worsening in the arthroscopy group at 12 months versus the nonsurgical group.  

Here’s the uncomfortable summary…

 

  • Imaging findings (cam/pincer shapes and labral tears) are common — and don’t consistently predict pain.
  • Hip impingement tests can produce false positives at a high rate — meaning “positive” isn’t the same as “proof.”
  • Injections don't predict surgical success.
  • Surgical outcomes are highly variable and high numbers of patients do not have their expectations met. 

So Why Doesn't Conventional Physical Therapy Always Restore Full Hip Function?

Movement and exercise play a crucial role in hip comfort and function.

But many people with hip impingement finish physical therapy appointments feeling like something is still missing.

Often, the focus is on 'protecting the joint' rather than progressively rebuilding strength, mobility, and function around it.

Exercises may stay in limited ranges. Resistance for large muscles may stay very light (if there is any at all). And deeper stabilizing muscles — like the hip rotators — sometimes get zero targeted training.

Combined with short, limited appointments and standardized protocols that may not address a person's unique issues, it becomes easier to see why some people experience limited or temporary improvements.

Without fully restoring muscular function, hips may still feel limited — even after therapy ends.
